SACROCOLPOPEXY OF A SIGMOIDNEOVAGINA

Introduction: A sigmoidneovagina is particulary challenging as it comes to surgery forprolpase due to special anatomical situation and the previoussurgical interventions performed.
Case Report: In thisvideo we present a sacrocolpopexy of a sigmoid neovagina with mesh.In a 41 year old woman a recurrence of a prolapse of a sigmoid vaginaoccured only a few month after a laparoscopic sacrocolpopexy withoutmesh was performed. Due to a botryoid sacroma at the age of 2 ahysterectomy and colpectomy had to be performed. At the age of 20 asigmoid neovagina was placed. In order to treat the recurrentprolapse a laparoscopic sacrocolpopexy with mesh was performed asdemonstrated in this video.
Conclusion: To be able toperform a sacrocolpopexy in a sigmoid neovagina, the technique tocreate such a neovagina has to be well known to the surgeon as it iscrucial to spare the vascular pedicle of the neovagina. Furthermore,adhesions and scarry tissue are common which also requires anexperienced surgeon.
This video shows, that a sacrocolpopexy in asigmoid vagina is feasible, an abdominal access is necessary. It iscrucial to spare the vascular pedicle to avoid necrosis of theneovagina.
